Start with documents and trusted contacts

A transition plan is easier to use when essential documents and reliable contacts are organized before an urgent need arises. Keep originals secure and maintain protected copies that can be reached without relying on one device.

  • Government-issued identification, Social Security card, birth certificate, and immigration documents when applicable
  • Health-insurance information, medication list, school records, and disability accommodation records
  • Names and current contact details for caseworkers, attorneys, mentors, relatives, and service providers
  • A calendar of court dates, benefits renewals, application deadlines, and important appointments

Build a housing plan with backups

Write down where you expect to live, what it will cost, who controls the lease, which utilities are included, and what happens if the arrangement ends. A backup plan should identify at least two people or agencies to contact before a situation becomes an emergency.
